Please print up and review the top three documents and fill out the "insurance billing" and "clinical questions" forms. This will decrease the time we spend on paperwork during our first session.  

 

Then, please bring in this packet  to your appointment where we can discuss any questions you may have and sign an agreement to  start working together.
